As disease epidemiology evolves and new evidence emerges, countries are increasingly reassessing vaccination timing, dose numbers, and booster strategies to improve long-term protection.

Recent evidence highlights the need for enhanced mumps control through adolescent booster vaccination. Although two-dose MMR schedules provide strong initial protection, immunity against mumps wanes more rapidly than for measles or rubella. Modeling studies suggest that booster doses administered during adolescence, particularly at 13–14 years of age, could substantially reduce transmission and prevent outbreaks among adolescents and young adults. These findings support consideration of age-specific booster strategies to sustain immunity.

For hepatitis B, current evidence indicates that routine booster vaccination is unnecessary for immunocompetent individuals who complete the primary series and achieve an adequate immune response. Long-term protection is maintained through immune memory even after antibody levels decline. However, optimized approaches—including booster doses, higher vaccine doses, or adjuvanted vaccines—may improve protection in populations with reduced vaccine responsiveness, such as healthcare workers, transplant recipients, people living with HIV, and individuals with obesity or chronic liver disease.

Evidence for varicella vaccination consistently favors a two-dose schedule. Compared with a single dose, two doses provide higher effectiveness against both overall disease and breakthrough infections. Countries that have incorporated two-dose varicella vaccination into routine immunization programs have achieved substantial reductions in disease incidence, hospitalizations, complications, and deaths. Recent data from China further support the effectiveness of this strategy. Although concerns have been raised about a possible shift of infection to older age groups, real-world evidence from countries with high vaccination coverage has not demonstrated this effect.

Overall, emerging evidence underscores that immunization schedule optimization should be evidence-based and disease-specific. Adolescent boosters may strengthen mumps control, targeted strategies are most appropriate for hepatitis B, and two-dose schedules offer the greatest benefit for varicella prevention. Continuous evaluation using surveillance and real-world effectiveness data will be critical for informing future immunization policy and maximizing public health impact.

Relevant Research and Evidence Updates:

[1] Wang, Y., Yang, T., Guo, X., Wu, Y., He, Y., Wang, L., Zhao, Z., & Chen, T.
