Although rotavirus vaccines have substantially reduced disease burden worldwide since their introduction, vaccine performance varies considerably across settings, creating challenges for countries considering vaccine adoption.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A recent study published in <em>eClinicalMedicine<\/em>&nbsp;developed a meta-regression model as a&nbsp;country-specific&nbsp;predictors&nbsp;of rotavirus vaccine performance and estimate the relationship between efficacy and effectiveness.&nbsp;Drawing on evidence from 100 studies across 56 countries, the researchers used a Bayesian meta-regression model to estimate vaccine performance based on readily available national indicators, including childhood diarrhea prevalence, sanitation coverage, population density, oral polio vaccine coverage, and antibiotic use.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The analysis identified a clear geographic pattern. Predicted vaccine efficacy was highest in Europe, the Americas, and the Western Pacific, where median estimates exceeded 80%. In contrast, lower efficacy was predicted in Africa and Southeast Asia, reflecting the greater challenges faced by high-burden settings. Nevertheless, substantial variation was observed even within the same region, highlighting the importance of country-specific assessments rather than broad regional assumptions.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A key finding was that vaccine effectiveness in real-world settings does not always mirror efficacy observed in clinical trials. In some countries with relatively low predicted efficacy, effectiveness remained moderate or even higher than expected. This may result from indirect protection through reduced community transmission and the accumulation of natural immunity among children. These findings suggest that lower efficacy estimates alone should not be used to dismiss the potential public health value of rotavirus vaccination.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The study provides an important decision-support tool for countries lacking local vaccine performance data. By generating country-specific estimates, policymakers can better evaluate whether vaccine introduction would be cost-effective when considered alongside disease burden, vaccine costs, and programmatic factors. At the same time, the authors caution that national-level predictions cannot fully capture subnational differences and should be interpreted alongside local surveillance data and expert input.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To summerize, this research represents a significant step toward more equitable and evidence-informed vaccine policy.
