In 2025, Lawrence R. Stanberry and colleagues advocated for adding a fourth indicator\u2014\u201cDrivers of Vaccine Uptake\u201d\u2014to complement the existing metrics. This addition would shift the paradigm from a \u201ccoverage-first\u201d to a \u201cbehaviour-driven-first\u201d approach, crucial for realizing the 2030 Immunization Agenda.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Limitations of the Existing Assessment System<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Presently, the JEE relies on three indicators: routine immunization coverage (e.g., measles first-dose coverage), national vaccine access and service delivery (e.g., cold-chain integrity, vaccination site distribution) and emergency response capacity for mass vaccination.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">These metrics focus on the operational \u201chardware\u201d of immunization programs while neglecting the \u201csoftware\u201d\u2014the behavioural and social factors such as safety concerns, accessibility issues, and social norms that drive vaccine uptake. During COVID-19, these overlooked drivers compounded supply issues, leading to inadequate coverage that static data alone could not explain.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>The Necessity for a <\/strong><strong>\u201cFourth Indicator<\/strong><strong>\u201d<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A \u201cDrivers of Vaccine Uptake\u201d indicator, based on the WHO\/UNICEF 2022 <em>Behavioural and Social Drivers of Vaccination: Tools and Practical Guidance<\/em>, would systematically assess four dimensions:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Thoughts and Feelings<\/strong> (cognitions and attitudes)<\/li>\n\n\n\n<li><strong>Social Processes<\/strong> (group norms)<\/li>\n\n\n\n<li><strong>Motivation<\/strong> (willingness to vaccinate)<\/li>\n\n\n\n<li><strong>Practical Issues<\/strong> (accessibility, affordability, service quality)<\/li>\n<\/ul>\n\n\n\n<figure class=\"wp-block-image size-full is-resized\"><img fetchpriority=\"high\" decoding=\"async\" width=\"711\" height=\"333\" src=\"https:\/\/vaxlab.dukekunshan.edu.cn\/wp-content\/uploads\/2025\/04\/\u622a\u5c4f2025-03-24-17.16.11.png\" alt=\"\" class=\"wp-image-14331\" style=\"width:839px;height:auto\" srcset=\"https:\/\/vaxlab.dukekunshan.edu.cn\/wp-content\/uploads\/2025\/04\/\u622a\u5c4f2025-03-24-17.16.11.png 711w, https:\/\/vaxlab.dukekunshan.edu.cn\/wp-content\/uploads\/2025\/04\/\u622a\u5c4f2025-03-24-17.16.11-300x141.png 300w\" sizes=\"(max-width: 711px) 100vw, 711px\" \/><figcaption class=\"wp-element-caption\"><strong>Fig.1<\/strong> The Model of Behavioural and Social Drivers (BeSD)<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">By employing standardized tools to collect and analyze behavioural data, this indicator would reveal the underlying causes of low coverage. Integrating this fourth indicator would enhance the overall JEE framework by: 1) <strong>Identifying Coverage Gaps, 2) Optimizing Communication, and 3) Enhancing Emergency Response.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To effectively implement this indicator, countries must establish robust data systems.
